Thank you for completing the 2017-2018 MHS registration form.  The online Google Registration Form link is available in this email and posted on the school webpage and Facebook. Student registration fees and any fines are posted online at Parent Connect under the fees link.  All demographic information updates are requested through the Parent Connect website.  Finally, any online credit card payments for registration fees, athletic participation fees, and lunch accounts may be made at

Online Registration Forms

Parents are asked to complete the online registration form survey listed below or online. Information is listed in five sections. Section 1 is the form completion and will serve as a legal signature. We ask that Emergency Contact and demographic information updates or changes are made through Parent Connect. Section 2 is a welcome survey. Section 3 will have the Health History Form. Mrs. McConville has updated requirements and links available on the School Nurse link at Section 4 references transportation. Due to the location of Mendota High School, every student is eligible to receive free bus transportation to and from school. Finally, Section 5 will cover the Athletic registration requirements. If parents would prefer a paper copy of the Athletic forms, they may receive forms at the Parent Athletic meeting on Sunday, August 6, 2017 at 6:00 PM in the gym. The updated MHS Student Athletic handbooks and forms will be available online at, Student Activities, and Athletic tab.

    2. The price for a full lunch is $2.75. Parents may view all lunch expenses used through EZSchoolpay.

  5. Online or pre-payment of student pictures option:

    1. Picture day #1 is July 26th from 10:00 AM to 6:00 PM.  It is also the on-site registration date with offices open until 6:00 PM.  Mr. Masini will be working this day to arrange payment plans for registration fees and/or process free and reduced lunch forms if needed.

    2. Picture day #2 is August 14th from 3:00 PM to 7:00 PM.  All freshman are asked to attend this day for the Freshman Orientation date.

Here are a few important dates and notes for you:

  1. The MHS registration form (Google Form) has a feedback option so you may receive an email confirmation at the end of the form.

  2. The MHS registration form allows you to also complete athletic authorization forms if you choose.  **The mandatory Athletic Parent meeting is Sunday, August 6th at 6:00 PM in the gym.

  3. All demographic information and emergency contact information may be updated online.  You need to log into your Parent Connect account using the parent student ID login.  The student ID login ends in the letter “s” for student, and the parent login either has no letter or “N”.  ONLY the parent login will allow demographic and emergency contact requests.

  4. Login into Parent Connect at  

    1. Click on the link under the student schedule in Parent Connect that says, “Request changes to your demographic information.”

    2. Make changes to phone numbers, email, and emergency contact information as examples. Once you submit your changes, an office staff member will approve the request.  No requests will automatically update without an office staff member’s approval. You must be logged in as a parent and not a student to make demographic change requests.


    4. See your registration fee amount due by clicking on Tuition/Fee Processing tab to the left of demographic information in Parent Connect. The registration fee is $90, and any lab fee due is $20 per class.  Lab fees are assigned for art, band, industrial arts, and food classes. The driver education fee is $100. Area Career Center students and fees have already been mailed to parents. **The athletic participation fee is not listed, but the fee is $55 per sport/activity with a $165 cap per student per year. All students participating in extracurricular activities are required to pay a participation fee.  Families with financial hardships or limited income could be exempt from paying both registration and participation fees, based upon qualifying for free and reduced lunches. Families with extenuating circumstances should contact either Assistant Principal, Mr. Masini, or Principal, Mrs. Aughenbaugh for a review of their situation. These situations are considered on a case-by-case basis and require documentation of need. **Families who are already pre-qualified to receive Free and Reduced lunch and fee waivers through the State of Illinois will show the registration fee is waived (zero) when reviewing student records in Parent Connect. Again, if you prefer in-person registration it is on  July 26th from 10:00 AM to 6:00 PM.  It is also the on-site registration date with offices open until 6:00 PM.  Mr. Masini will be working this day to arrange payment plans for registration fees and/or process free and reduced lunch forms if needed.

  5. HR Imaging will be at Mendota High School on July 26th from 10:00 AM to 6:00 PM to take Senior photos and ID/Yearbook pictures for sophomores, juniors, and seniors.  (If you miss the July 26th date, please have your photo taken on August 14th.) In-person or on-site registration is also available on July 26th from 10:00 AM to 6:00 PM.  Parents may pay registration fees in cash, or parents may have access to a computer to complete the online registration form.  (**We ask all freshman attend the Freshman Orientation session on Monday, August 14th. A separate mailing has been sent to those families.) HR Imaging photographers will be available on August 14th from 1:00 to 7:00 PM to again take ID and yearbook photos for any student or staff member.  

    1. Students were mailed a picture packet from HR Imagining in June to the address currently on file at school.

    2. If you did not receive a picture packet you may order pictures on either July 26th or August 14th with the photographer, and you may also prepay and preorder online at

    3. **Students are not required to purchase pictures, but they are all required to have a photo taken for student ID and yearbook purposes.  

    4. Any senior who does not have his/her picture taken will not have a photo available in the spring graduation announcements.

  6. SENIORS are required to have the MCV immunization.  More information is available on the nurse’s web page at

  7. All sophomores, juniors, and seniors planning on participating in a sport must have an athletic/sport physical prior to participating.  

The first day of school for students is Wednesday, August 16th.  Students report to the gym at 8:05 to open with an all-school assembly.

Area Career students attend the first day of school on Monday, August 14th.  ACC students are excused from the Area Career Center on Wednesday, August 16th so they may attend the opening assembly and homeroom at MHS.


Mendota High School’s accident insurance policy covers your child for injuries that the student might receive while at school or while participating in athletics secondary to your personal, primary insurance. (The family insurance is billed first before any claims may be submitted to the secondary school insurance.) **Additional, 24 hour coverage is available for purchase by parents. You may request an insurance brochure or information in the Principal’s Office from Mrs. Biers.
